PECOS Revalidation: Step-by-Step

Medicare requires enrolled providers to revalidate their enrollment every 5 years (3 years for high-risk providers). Revalidation in PECOS is essentially an update and confirmation of your existing enrollment record — but it’s a formal process that must be completed by the deadline or your billing privileges are deactivated.

Step 1: Check your revalidation due date

Before doing anything else, confirm when your revalidation is due.

Option A: PECOS lookup Log in to PECOS (pecos.cms.hhs.gov) and navigate to your enrollment record. Your revalidation due date is listed in your enrollment status section.

Option B: CMS revalidation lookup tool CMS has a public revalidation lookup tool at: https://pecos.cms.hhs.gov/pecos/login.do → “Check Revalidation Due Dates” (available on the PECOS public landing page)

Enter your NPI to look up your due date without logging in.

Option C: Contact your MAC Call your MAC’s provider enrollment line and ask for your revalidation due date.


Step 2: Confirm your correspondence address is current

Before revalidating, check the correspondence address on your enrollment record. Your revalidation notice — and any development requests — go there. If it’s outdated, update it as part of the revalidation.

In PECOS: navigate to your enrollment record and find the correspondence address section. Update if needed before initiating revalidation.


Step 3: Initiate revalidation in PECOS

Log in to PECOS. Navigate to your enrollment record (individual or organization).

If your revalidation is due, PECOS may show a “Revalidation Due” prompt or banner. You can also initiate revalidation manually from your enrollment record.

Click “Revalidate” or “Submit Revalidation” to begin.


Step 4: Review and update each section

PECOS will walk you through each section of your enrollment, pre-populated with current data. Your job is to:

  1. Review each field
  2. Update anything that has changed
  3. Confirm everything that’s still accurate

Section-by-section review:

Identifying information

  • Confirm your legal name matches your NPI record
  • Confirm your NPI is correct
  • Confirm your SSN (or EIN for organizations)

Practice location(s)

  • Verify each location address is still accurate
  • Remove locations where you’re no longer practicing
  • Add any new practice locations
  • Verify phone numbers and fax numbers

Correspondence address

  • Verify this is an address where you reliably receive mail
  • Update if needed — this is the most important update in revalidation

Licensure

  • Update expiration dates for licenses that have renewed
  • Add new licenses (new states, additional credentials)
  • Remove expired licenses
  • All active practice state licenses must be current

Malpractice insurance

  • Update to current policy (most malpractice policies renew annually)
  • Enter current policy number, effective date, expiration date, coverage amounts

Ownership and managing control (organizations)

  • Confirm all current owners and their ownership percentages
  • Add any new owners
  • Remove any departed owners
  • Update managing employees

Adverse legal history

  • Disclose any adverse actions since your last enrollment filing
  • This includes anything that happened since enrollment, not just since the last revalidation

Step 5: Upload supporting documentation

For revalidation, you may need to upload updated documentation:

  • Current state license copy
  • Current malpractice insurance certificate
  • Any other documentation that has changed or expired since last enrollment

PECOS typically doesn’t require you to re-upload documents that haven’t changed, but attach updated versions of time-sensitive documents (licenses, malpractice certificates).


Step 6: Certify and sign

Review the complete revalidation application summary. Certify that all information is accurate, current, and complete.

Sign electronically with your I&A credentials.


Step 7: Submit and track

Submit the revalidation. Note your application ID.

Track status every 30 days. Most revalidation processing completes within 30–60 days via PECOS.

If your status shows “Pending Completion,” your MAC has sent a development request. Check your correspondence address immediately and respond within 30 days.


⚠️ Risk flags

⚠️ Filing late is the same as not filing. If your revalidation is due December 31 and you submit December 28, but it takes 60 days to process — you’re okay, the submission date is what matters, not the approval date. But don’t wait until the last week.

⚠️ Organizations and individual providers revalidate separately. A physician employed by a group has two revalidation requirements: their individual CMS-855I enrollment and the organization’s CMS-855B enrollment. Both must be current. Check both on the same schedule.

⚠️ Revalidating without updating means outdated information stays in the system. Don’t just click through — actually verify. A revalidation that confirms an incorrect practice address or outdated license just propagates the error for another 5 years.


After revalidation approval

You’ll receive a confirmation letter at your correspondence address. Your next revalidation due date resets from the approval date.

No action needed after revalidation is approved — your existing PTAN and enrollment status continue.
