TL;DR — For most providers, PECOS beats the paper CMS-855I: faster processing (often 30–45 days versus 60–90), real-time status tracking, and pre-filled revalidations. Paper remains the right call only in a handful of situations covered below. Most providers have a choice: file your Medicare enrollment electronically through PECOS (the Provider Enrollment, Chain and Ownership System) or submit a paper CMS-855I to your MAC. The answer for most providers: use PECOS.
Here’s why — and when paper is still the right call.
The case for PECOS
1. Speed Paper CMS-855I applications typically process in 90–180 days. PECOS electronic submissions average 60–90 days. For many specialties, that’s a month or more of earlier billing eligibility.
2. Status tracking PECOS shows you where your application is in the review process. Paper gives you nothing until you hear from the MAC — which might be a development letter asking for more information 60 days in.
3. Electronic signature PECOS allows electronic signature. Paper requires wet-ink signature, mailing logistics, and risk of a signature being missed or illegible.
4. Pre-populated data from NPPES PECOS pulls your NPI and NPPES data automatically. This reduces transcription errors in name, address, and taxonomy.
5. MAC preference Most Medicare Administrative Contractors prefer electronic submissions and process them faster. This isn’t officially stated as a priority, but processing time data consistently shows electronic submissions process faster.
6. Easier change of information Once enrolled, you can make changes to your enrollment record (address updates, adding practice locations, changing correspondence address) through PECOS without submitting a new paper form.
The case for paper
There are legitimate situations where paper is required or preferable:
1. CMS-855 forms that don’t have a PECOS equivalent Some enrollment-related forms must still be submitted on paper:
- CMS-588 (Electronic Funds Transfer) — paper only
- CMS-460 (Participating Physician Agreement) — paper only
- CMS-2728 (ESRD Medical Evidence Report) — paper only
- Opt-out affidavits — paper only
2. PECOS system outages or access problems PECOS experiences periodic maintenance and outages. If your enrollment is time-sensitive and PECOS is unavailable, paper is your fallback.
3. Specific change requests not available in PECOS Some complex enrollment changes (certain reassignment scenarios, specific change of information requests) may need to be handled on paper per MAC instruction.
4. DMEPOS suppliers DMEPOS enrollment via CMS-855S is handled by the National Supplier Clearinghouse (NSC) and is a paper-based process (PECOS is not available for CMS-855S).
5. Institutional providers (CMS-855A) Many institutional enrollment scenarios are not fully available through PECOS. Check with your MAC.

PECOS setup requirement
To file via PECOS, you need a CMS Identity & Access Management (I&A) account linked to your NPI. If you don’t have one, you’ll need to create it before you can use PECOS — the setup takes 24–48 hours for identity verification.

If PECOS account setup creates a timeline problem for a time-sensitive enrollment, submitting paper while setting up PECOS in parallel is a reasonable approach. Talk to your MAC.
⚠️ Risk flags
⚠️ Don’t start paper if you can use PECOS. The 30–90 day processing difference is real. Starting with paper when you’re eligible for PECOS just means later billing authorization.
⚠️ Don’t double-submit. If you start in PECOS, don’t also send a paper form “just in case.” Duplicate submissions confuse the MAC processing queue and can actually slow things down. Pick one path.
⚠️ Current form version. If you do submit paper, use the current version of the CMS-855I from cms.gov. MACs reject outdated form versions. The current version number is printed on the form footer.
Recommendation
For individual provider enrollment (CMS-855I) and organization enrollment (CMS-855B): use PECOS. Set up your I&A account, verify your NPI information in NPPES, and file electronically.
For CMS-588, CMS-460, and opt-out affidavits: paper is your only option regardless.
For DMEPOS (CMS-855S) and most institutional enrollment (CMS-855A): follow NSC or MAC instructions, which are primarily paper-based.
